Alternative Cares’ Specialist Support Coordination will assist in resolving complex barriers and build sustainability in your relationships within the home and community.
Our staff have the qualifications and extensive experience to assist you to:
· source positive behaviour service providers where necessary;
· monitor the use of restrictive practices;
· ensure all reporting requirements are completed by service providers;
· address communication barriers;
· develop self-awareness and understanding during crisis;
· establish effective strategies for crisis prevention;
· promote longevity of your self-directed accommodation service agreement;
· promote resilience within your co-residency relationships;
· promote safety for yourself; and
· promote the safety of others
Our Specialist Support Coordinators work closely with participants and their self-directed decision makers to address the complex needs and barriers by ensuring available supports funded by the NDIS have been considered within the planning process.
Participants and their decision makers are supported in reviewing the capacity building achievements of the plan. This review enables the identification of any additional supports required to achieve the goals within the NDIS plan. This review ensures all high-risk environments are reviewed on a quarterly basis at a minimum.
Our Specialist Support Coordinator will ensure:
· you are provided with a plan that clearly identifies all obligations of service providers to address complex barriers;
· you are supported with the ongoing review funding arrangements;
· an appropriate Positive Behavior Practitioner is sourced for the development of a positive behavior support plan;
· funding requests are made to ensure supporting staff are appropriately trained in positive behavior support strategies and restrictive practices legislation;
· services and support are coordinated to meet complex needs and environments;
· transparent, respectful, and honest communication processes are ongoing with participants, their self-directed decision makers; and
· allied health professionals work with the participant and others collaboratively.
