Alternative Care will support you in achieving your goals and self-directed needs within the home environment. Our staff will ensure all service provider obligations detailed within your Service Agreement are met.
Alternative Care’s Support Coordinators may support participants by visiting them within their homes to ensure a holistic living environment is maintained and appropriate safeguards are in place.
Our Support Coordination planning and review processes include a participants’ choice of decision makers, support networks, and service providers.
With our staff experience, we are able to work towards alleviating the stress support participants and decision makers to have the concerns addressed.
How we can assist with your NDIS plan
· Explain the possibilities available within your NDIS plan
· Assist in the ongoing funding and affordability of service required within the total funding agreement
· Ensure participant self-direction
· Explore a choice of service providers to meet your needs
· Explore a choice of supports and assistive technology to promote independence
· Effectively monitor the service provision
· Work towards developing, maintaining, and strengthening social and community relationships
· Negotiate and finalize Service Agreements with other service providers
· Ensure resiliency and sustainability of chosen service providers
· Negotiate funding to address the need for Specialist Support Coordination to address harm, points of crisis, or the deterioration of service agreements.
